In case of medical emergency, I understand every reasonable effort will be made to contact me. In the event that I cannot be reached through reasonable efforts, I hereby give permission to the physician selected by the Summer Camp Director, Scott Manley, to secure proper treatment of, to hospitalize, order injections, anesthesia, or surgery for my child. As the parent/guardian, I further agree that I will not hold Olympia Chinese Christian Church, the facilities, their agents responsible for any accident or injury.
